| Who is the Cover for
You can obtain cover for a Single person, a Married Couple (without children), a Family (couple with children) or if you are a Single Parent. |
|
| Number of Children
If you select Family or Single Parent please tell us how many children you have - there is no limit to this number. Your children must be dependent on you and must be under age 21 (or under age 25 if still in full-time education). |
|
| Eldest Applicant
Please let us know the age of the Eldest Applicant to be covered by this policy. The maximum age we accept for dental insurance is 74. |

| Cover Start Date | |
| Pre-existing Conditions
For the first two years of cover, your dental insurance policy with us excludes treatment for illness, injury or condition which existed at any time in the five years before you took out the policy (pre-existing conditions). This exclusion is known as a moratorium clause. Specifically the exclusion of pre-existing conditions states: During the first 24 months of insurance, benefit is not payable under this policy for treatment for any pre-existing condition or related condition. This is known as the moratorium period. Pre-existing conditions and related conditions will be covered after the moratorium period if you have not received any medical advice or treatment for such conditions during the moratorium period. If at any time during the moratorium period you receive any dental advice or treatment for a pre-existing condition or related condition, a new two-year moratorium period for that pre-existing condition and related condition will start from the date of the latest advice or treatment. |
| Non-emergency dental treatment
Cover for non-emergency dental treatment is not provided until your dentist has completed and returned your Certificate of Dental Health, which is sent to you with your policy documents. |
Please note that Dental Care is issued and administered by our sister company, Universal Provident Limited, and is underwritten by Personal Assurance Plc.
Dental Care is a one year dental insurance contract that provides cover against the cost of both emergency and non-emergency dental treatment at an affordable premium. We have been able to keep premiums as low as possible by restricting the cover so that we do not pay for the cost of routine treatments such as check-ups and replacement fillings.
Your dental insurance has generous limits provided for many other types of dental treatment and your children are FREE. Read the Policy Wording and the Policy Summary for more details.
